Although managed health care is a hot topic, too few discussions focus on health care rationing--who lives and who dies, death versus dollars. In this book physician and bioethicist Peter A. Ubel argues that physicians, health insurance companies, managed care organizations, and governments need to consider the cost-effectiveness of many new health care technologies. In particular, they need to think about how best to ration health care. Ubel believes that standard medical training should provide physicians with the expertise to decide when to withhold health care from patients. He discusses the moral questions raised by this position, and by health care rationing in general. He incorporates ethical arguments about the appropriate role of cost-effectiveness analysis in health care rationing, empirical research about how the general public wants to ration care, and clinical insights based on his practice of general internal medicine. Straddling the fields of ethics, economics, research psychology, and clinical medicine, he moves the debate forward from "whether to ration to "how to ration. The discussion is enlivened by actual case studies.

Nursing, Medicine and Primary Care by Anne Williams,
Nursing, Medicine and Primary Care by Anne Williams,
The juxtaposition of medical and nursing cultures is nowhere more evident than in primary care, the scene of recent and critical changes an policy which ostensibly mark a departure from competitive market-oriented system of health care to one which promotes partnership and collaboration. Based on recent research, this book provides a broadly sociological analysis of shifting boundaries between and within the profession of medicine and nursing in the context of primary care. The analysis highlights ideas which are current within the contemporary health services -- ideas about enterprise as well as meeting need and empowering patients. The ways in which ideas are used to justify role change, and to establish and maintain professional identities are discusses. A cornerstone of the book's argument is an exploration of the scale and nature of uncertainties and vulnerability experienced by nurses and doctors in relation to boundary changes. An analysis of the problems as well as the opportunities which arise as a consequence of working in a climate of uncertainty is a key feature of the book, making it directly relevant to a wide range of health care professionals. Nursing, Medicine and Primary Care has also been written with a student audience in mind and will be of interest to undergraduates and postgraduates in areas such as; nursing, medicine, health studies, the sociology of work, public sector management, medical sociology and women's studies.
